- When you fill out a voter registration form, we will request your full name, email address, street address, phone number, and date of birth.
- If you are updating a previous voter registration, we will also ask you for the city, state, county and zip code of your previous residence or your former name.

How We Use and Share Information
We use the personal information that we collect from you to provide the Service. Once you have provided all the requested information, we will generate a new or updated voter registration form and mail or email it to you. We do not sell your information.
